Mondays, Tuesdays, Thursdays & scheduled Saturdays.
10:00am to 5:00pm + evening appointments
After work and evening appointments are also available to help accommodate your busy schedule. We’ll be adding
more weekday openings shortly—check back at the beginning of every month for the latest schedule to see the dentist.

New Patient Forms

Here you can find all the new patient forms to fill in so that the dentist is aware of any dental or health conditions or concerns you may have.
This information will help the dentist determine what treatment options are available and suited to you.
Payment Policy & Insurance Claims
At your first appointment, we will enter your insurance information into our system. This allows us to submit your insurance claim electronically,
expediting the process for you.
After your visit, we electronically send your claim to your insurance company on your behalf. Reimbursements can be received as quickly as the next day
or within five business days, depending on your arrangement with the insurer.
Payment in full is required at the time of service and can be made with Debit, VISA, or MasterCard.
For subsequent procedures, we will send predeterminations to your insurance company before treatment and explain their response to keep you updated.
