Where Are We Located?
We’re located in Stoney Creek within Hamilton Ontario at the corner of Winona Road and Hwy 8/Queenston Road. The nearest exits off of the QEW would be either Fifty Road and Fruitland Road.

New Patient Forms
Here you can find all the new patient forms to fill-in so that the dentist is aware of any dental or health conditions or concerns that you may have.
This information will help the dentist determine what treatment options to recommend for your specific health needs.
Payment Policy & Insurance Claims
At your first appointment, we will enter your insurance information into our system. This will allow us to submit your insurance claim electronically which will expedite the process for you.
At the end of your visit, we will electronically send your insurance claim for your procedures to the insurance company on your behalf. The insurance company will then reimburse you for the amount you are covered for. Reimbursements can be received as quickly as the next day or within five business days depending on the arrangement between you and the insurance company.
Payment in full is required at the time of service and can be made with Debit, VISA or MasterCard.
For subsequent procedures, we will send off predeterminations to your insurance company before treatment and explain their response so that you are up to date at the time of your dental treatment.
